Help finding a form

I am looking for Geralex time-off request form. Can you help me find it?

my mother works for Geralex. she was given this link: https://form.jotform.com/geralex/time-off-request 

When I open that link I get a message "Unfortunately, the page you were looking for could not be found. It may be temporarily unavailable, moved or no longer exists. Please check your spelling and retry."

Can you help me find that form so I can fill it out for my mom? 

I think you are referring to the link https://form.jotform.com/geralexinc/time-off-request which doesn't seem to be active. Could you check the link once again and let us know so that we can assist you further?
