Adding Multiple Rows and Columns to Forms

I wish to add 3 Columns with multiple rows (as needed) that workers can type into on-line 

Columns I wish to label myself

No title rows like 1, 2, 3

All rows must stay together and not move around. Almost like a xcell sheet
