WOA is a non-profit organization with chapters in many states within the US. We had our form go out in the first quarter. when someone used our link, it ended up with a different chapter.
how could this have happened?
how can it be avoided?
Please help.

I've checked all your recipient form notifications and see only this email: bk377t@att.com, that means it should be the only email will receive a notification if someone has filled out your form.
Could you let us know or tell us more details of the problem you mentioned?

Sorry for the late response. I am the owner for the link, all responses but this one came to me.
This was the link used by the person https://form.jotform.com/woawa/woa-wa-scholarship-application-2018
I have not received the email (the one I am talking about); it went as a response to another form
The email address for the misplaced form is chowdharys2@gmail.com . The person did not have a link to the other form that she ended up submitting. Do let me know if you need any other information.
Please help me understand the mystery.

I am not sure that I have understood your question correctly, are you saying that the Form https://form.jotform.com/woawa/woa-wa-scholarship-application-2018 when submitted went to this email Id chowdharys2@gmail.com?
I checked the Form, I could not find any discrepancy in the email notification and autoresponse email. It may be possible that the email chowdharys2@gmail.com filled up your Form and an autoresponse email went to this Id.
If possible please re-add the autoresponse and notification email. Also, you may clear your Form cache.
