Form is not working in Non-profit Pricing page.

I tried to submit my information @ https://www.jotform.com/nonprofit/pricing/

But the form is not working.  How can I submit my request for non-profit pricing?

I test submitted the form in non-profit pricing page https://www.jotform.com/nonprofit/pricing/ and It works fine on my end. See my screencast below.
