HIPPA/PHI Compliant Forms with Salesforce Integration

    We use an employee information form to collect information from individuals that is considered PHI for health insurance purposes.  

    We are looking for a form builder that we can utilize to collect this information and automatically upload to our salesforce platform.

    Essentially we want to be able to send an employer a link that they will share with their employees.  The employees will log in to the form and complete all information.  Once they employee submits the form, we want it to connect to our salesforce platform and create a new contact with all the information provided by the employee.

    We would like to have a form interactive...if the employee states that they are married, then additional fields pertaining to their spouse would be available to them, but if they are single, they will not have to deal with the spouse questions.

    Can you advise if this is something that you system can assist with and the cost to be able to do so?

    We do offer HIPAA compliant forms:

    However, the Salesforce integration is not HIPAA compliant so it is not currently included when using HIPAA compliant forms.  If Salesforce integration is a must, our HIPAA compliant forms unfortunately would not suit your needs.

    If I am asking in the wrong location, please point me to the right area to answer my questions.


    So based on the information, the forms themselves and the storage of the data upon form submission meets HIPPA compliance, but once we try to integrate another system, Salesforce/Google Sheets etc..., the compliance goes away.  How would we get the information from JotForm?  Would the info all be dumped into a spreadsheet that we can export, or would the results be delivered to an email address for each form completed.  Trying to figure out how I can obtain the information and import it into our Salesforce platform that is HIPPA compliant.

    1. Google Spreadsheet is available on HIPAA account for integration. Here are other apps available on HIPAA account.

    2. Just like in regular account, you can also export your submissions data to Excel, CSV or PDF. Guide: https://www.jotform.com/help/73-How-to-Download-Form-Submissions-as-Excel-CSV-PDF

    3. By default, you'll also receive an email notification containing the form data on every submission.
