One form format somehow goes into the other form

I have 2 forms in my account When I fill in one form in browser everything is fine, the form is correct and all goes well. However when I submit the form, I get the PDf file and HMTL submission by email in a totally wrong way: the data I entered in browser is correct (though incomplete) but the fields are taken from my other form in account. I attach below two pictures showing the completed form in the browser and the PDF file which I got. 

Any ideas would be much appreciated!

It seems that you have cloned the form to create new one and then renamed the labels. PDF template was cloned along with the form, but labels are not updated automatically. I can suggest to reset PDF template by creating new one, you may then delete old one:
