Convert into Fillalable PDF Form

I already have prepared a form to be used online.  How do I convert it to a pdf fillable form.  I did this step last year and it was easy, but don't know how to do this now?

If you have an existing fillable PDF form, you can upload it to JotForm. Open this page https://www.jotform.com/pdf-editor?fillable=new and select Import PDF Form.
