Set up a value by my self and reuse it

Hello, 

I will have to send the same form to many contacts (lets say Guillaume, Max, John, Nicolas ...). It is exactly the same questions but I would like to customize the form with the name of Guillaume, Max, John ans Nicolas which appears around 30 times in the form. 

I would like to find a way to avoid a 30 time copy paste of Max, then John and Nicolas in each form questions

Is their a way to set a value by my self, the name of my contacts, i(n a hidden field for example) and then call the value as it is possible for the unique name in detail field ?

To achieve this, make use of the Spreadsheet to Form Widget. It will allow you to upload a set of preconfigured values such that when you fill in a specific name, all the data in the row of the Spreadsheet is automatically filled into the form in the right places.
