Doesn't it display after you embed the code in your site? If it doesn't, please try a different embedding method such as the Iframe one. If the issue persists, please share the URL of the webpage where you have embedded the code.

There is no embedded jotform on the main page: http://www.dustfairies.co.uk/

But, I found one on the contact page: http://www.dustfairies.co.uk/contact.html

The form is deleted, you may find it in the trash folder: https://www.jotform.com/help/134-How-to-Restore-Deleted-Forms

Unfortunately, we cannot give you access to that account or form. You need to contact the form owner to request transfer of ownership.

Of course, you can use your new form instead of old disabled one. To do so, please try replacing embed code on your web page. You may use this method: Adding-a-Form-to-a-Web-Page

    Or this: Getting-the-Form-iFrame-Code

It depends on the platform used for your web site. Can you tell us what platform (WIX, WordPress etc) your web site is running? We would be glad to check for instructions for you.
