prepopulating second form based on the first form data

is there a manual step by step how to populate the second form based on the first.

i tried the conditional for redirect page after submiting 

but there is no place to add the fields in it.

the request is to have a first page that gets the user information and based on its requests the form should divert him\her to the correct form. and in order to prevent the inconvinance of refilling the information again, we would like to prefill those
