Make sure you have created a list field in your mail chimp account. Go to your list name > Settings > List fields then add the desired fields to be linked in your form.
I tested a submission and I successfully received the submission in my MailChimp list.Please give it a try and let us know how it goes.

Thanks for your help. I have done this and now only some of my form
entries are appearing in MailChimp, and only after I go to JotForm
integrations and force a refresh?

You should add other fields on the integration.
Please click on Edit Integration first:
Then, you should click on Add New Field option:
You can add your required fields, then you should update the integration.
If the issue persists, please let us know.
We would be appreciated if you could share the field names that are missing.
