Card form resolution and appearance on phone is not accurate and bad

Intermittently, the form changes view on phones. It looks fantastic on laptop, but it is awful in the Phone view (at times). I have tested with different devices.  Not sure why it is doing what it is doing.  Please see my form in phone view. I have also included a screen shot. It happens (when it does) on the 3 or 4th slide. 

I have cloned and tested your form in form builder. I wasn't able to reproduce this issue.I can suggest you clear your form cache. It may cause this bug.
