We manage buildings and need to provide tenants with a notice to enter for maintenance work, even if the work is requested. I want to have all of the information pre-filled in the form and simply have the tenant approve the request with a signature.
The information I will need populated in individual fields includes: building name, tenant name, unit #, maintenance requested, date and time of entry, etc.
I want to give only one option and that is to approve the entry or not.
Secondarily, I would like all form data to export/save to a google sheet.
I don't know how to do any of this and would appreciate some advice. Thank you.

You can add all the necessary fields by using our basic fields:
Where your "date and time of entry" you can set with a Date field, and the rest of the data you could collect with Short Text Entry field.
Regarding Approve and Deny, I assume you would do this once they submit the form, so the option shouldn't be visible for the submitter?
If that's the case, you could insert a Single choice button:
And hide it:
When the form is submitted, you can edit the submission and set that field to what is needed:
