How can I create a word document?

I signed up for jorform but i think i don't get it how it works

Can i create a form for example a word questionnaire document??

Jotform is an online form builder where you can create an online form to collect registrations or signups. We do have a way to export the form as a PDF, but no option you can export forms as a Word file.

Thank you for the information. I would suggest creating an online version then because that gives you a more dynamic option to collect basic information from your clients. 

Your current form (https://www.jotform.us/form/90865273543160) is a good start. You only need to edit the form to add more fields in it. 

To receive the data, you can create notification email in it.

If you would like to send a copy to your clients, you need to create an autoresponder email.
