Form is not working

    Hello,

    I have created a questionnaire form. It allows to enter all the fields, but does not allow to submit. What I did wrong? It shows please wait and does nothing.

    I tested your form and it is working fine. I successfully submitted the form and I was redirected to the Thank You page.

    Please test the form again. If the issue still persists, let us know which device and browser are you using. 

    If the form is embedded provide us a webpage URL, so that we can inspect and test the form there.

    It appears that you have used wordpress embed plugin to add a form to your web page. It can only display the form, but does not provide functionality to submit forms. Please try embedding form using its iframe code and add it to web page post via TEXT mode. Here is how to get iframe code: Getting-the-Form-iFrame-Code
