    The user inputted data to my inquiries get into the PDF. When a client clicks YES to the question "are they married" a new input box pops up in the interview BUT that spouse's name does not make it into the PDF.

    Can you please let us know what is the form in question so we checked it closer?

    In general, if a field is missing, PDF template probably must be updated for your form. You can add a field manually or to reset a template. To reset, please create new PDF document and then you can delete old one, new template should capture all form fields fresh: How-to-Customize-PDF-Submissions-Report
