I can't find my form

I made a form when 27. Apr. 2019

and used the link(https://form.jotform.me/91154431594457).

But, the link isn't working now, and I can't find the form.

PLZ reply asap...

I did check your form #91154431594457 and found that it is associated with account "qetuop02" which is currently suspended.

Your account was suspended because of this form Cleaning Service Agreement Form asking credit card information. We have deleted the CC fields in your form and unsuspended your account.

Due to our PCI certification, we are required to deny the collection of credit card numbers using regular textbox fields. The only and proper way to collect and process credit card based on PCI compliant ways is to use any of the payment processors available in the form builder under the payments tab. You will have to integrate your form with the payment gateway of your choice like Paypal, Stripe, Authorize.net and etc. Through this payment integration, the form will process the credit card securely through the payment gateways API. 

Here’s how:

1. Integrate your form with any of our Payment integrations.
