I have uploaded my fillable PDF form but the output isn't the same as the form I uploaded

    I have uploaded my fillable PDF form but the output isn't the same as the form i uploaded.  Am I doing something wrong

    Could you please share screenshots to see the difference between the two files? Also, please uploaded your fillable PDF form to Google Drive, and share with us the shareable link, so we can download it to test in our end.

    The form output after uploading just has all the questions listed instead of in the actual form I uploaded as shown in the screen shots

    Please do the following:

    1) Log in to your account, and click on the Create Form button:

    2) Then click on the "All questions in one page" layout:

    3) Click on the Import button of the IMPORT FORM option:

    4) Click on Import PDF, and click on Continue:

    5) There upload your Fillable PDF form, you will preview the layout, and it will look the same:

    However, when you see it in the builder it will not look the same. It will show the original layout when the form seen through the direct link: https://form.jotform.com/91256946898983

    Note: when you start filling it, the web layout shows again, after you finish filling the form, you will click on preview, and the preview of the original form will show up with the filled data.

    Thank you that was helpful. 

    Next question it is not allowing me to enter additional fields that were missed on the upload
