I did not intend to disable HIPAA compliance, now I cannot see any of my payment information.

Your account is currently on a free plan. The HIPAA compliant forms are available with our Silver and Gold plans only.
In other words, to be able to use HIPAA compliant forms, you will need to upgrade to the Silver or Gold plan.
