Setting up gift certificate options on Jotform

Hi All -- how would I go about setting up a gift certificate to be sent by email/printed as the attached image shows? Thanks!

Is this a different from than the one in your account? I am not sure if I understand your query correctly. Do you need to create a Gift certificate code and add it to the email? If so, you may create one by customizing Unique ID widget on your form. Alternatively, you may use Random Value generator widget also. If you are already having a Gift certificate, you may customize the email autoresponder to send it to the user.
