how to customize google forms

Hey, I'd like to create registration form for my soccer practice team. Since mostly kids and their parents fill it out, I want it to be visually perfect and easy to fill out. I checked out google forms but couldn't find any details on design. I need your help with this please. Also, you can also tell me if I can do any custom design on jotform. Thanks in advance.
