Question about uploaded jpgs in submissions and how to access them?

    Hi, I am new to forms - just trying to figure out the process. We are looking to create a form for kids to sign up for youth group. Basic info plus emergency contacts, health data, etc. I'd like to include a place to upload a picture of their insurance card, front and back - so two files. Once I get the submissions I can download the data to excel, but what is the process to get the photos of the insurance cards to download? Thank you.

    A link to such image will be included in Excel when you export your submissions: https://www.jotform.com/help/44-How-to-Export-Form-Data-to-Excel

    What I would advise though is to check our Terms of use since you're planning to collect sensitive data, so you should be aware that collecting such data can be a reason for suspension.

    I checked the terms and it doesn’t say what is sensitive data other than credit card numbers. So how is a person supposed to know what is sensitive?

    Please take note also that in the Terms, it mentioned in the Collecting Sensitive Information clause, that it is not limited to credit card information only. 

    If you intend to collect sensitive information using the form, make sure to use best security practices.

    I had seen that part of the terms, my question is, what else is sensitive
    data? How do you know if you are collecting it if you don't list it out
    anywhere?

    Please note that username & password, email & password, SSN, driver's license, ID upload, using unauthorised logo image are not allowed. 

    We have automated ant-phishing filter which scans all the forms. Please get back to us on this after you create form, we will review it.
