I have checked your account and indeed you have switched from monthly to yearly, but I cannot find the double charges, I can see the following:
- You were charged May 28, 2019 for the monthly plan, this was the normal charge as you had that subscription active at that time.
- You then switched to the yearly plan on June 5, 2019 and were charged with $175.75 which is not the full yearly price as the unused time of the monthly plan was discounted.
So basically those charges are normal, you can check your invoices following this guide too: https://www.jotform.com/help/329-How-to-Check-Invoices
This way you may also confirm what I've explained.
Please kindly let us know if you're referring to different charges and kindly provide more details about it.
