Added take photo widget and when you send it says "please wait"

Also added take photo widget and when you send it says "please wait" but after 5 mins it was still on thst screen and didn't complete. Iam on a good Internet connection. Please advise?

Also, does the issue happen to form when accessing it via form direct link or you have embedded the form to a web page?

I found out the issue was the error with the pdf embedder. I couldn't
attach pdf as it said unsupported file. So left it blank. When i deleted
the embedder the photo attached fine
...
