Can I use multiple pages and have multiple input fields per page?

You can create a multi page form with JotForm. You can use either our classic or card form layout options.
If you choose using a classic form layout, you can divide your form into pages with page breaks. In order to add a page break, click the "Add New Page Here" button in the form builder.

In addition, you can create multi-pages while using card form layout. The aim of the card form is to show one question per page but you can add more than one form elements to one page with the "Multi-line Question" element as follows:
