Signatures on HIPPA Compliant forms

    Hi, 

    We are looking into Jotform for a medical practice. There are some documents that require the patient's signature. 

    It looks like there are widgets that allow patients to sign the form. We then need a copy of that form and signature on file to upload to the our Electronic Health Record (EHR) software. 

    Will Jotform send the signature on the form to us (in a pdf format) so we can then upload it into our EHR?

    Yes, that's possible with JotForm.

    Here's the Signature Widget that you can add to your HIPAA Form http://widgets.jotform.com/widget/signature.

    You can download the signature image files and a PDF of your submissions containing the signature image when you go to your form submissions page.

    You can also include the PDF as an attachment on your form notifications. Guide: https://www.jotform.com/help/503-How-to-include-submissions-as-PDF-attachment-in-the-email-notifications
