Issue with Configurable List

Hi I have a form which I wrote some time ago its called PI Report

Within this form I have an input table by selecting an "Action required" tab of the input table a configurable list is displayed where the issue can be logged. This all works fine and is displayed in the final pdf report, however if I have forgotten to enter something, go back to the page using the back buttons, select another area where action is required and add the data to the configurable list, it is not recorded and therefore does not appear in the final pdf report. Can you help please ? This used to work. Ive tried it on Mac in Safari, Chrome on a PC and iPhone. It happens exactly the same on edit after submission.
