We do not allow for the collection of driver Id and banking details through forms. Please refer to our terms below.

    https://www.jotform.com/help/8-Terms-of-Use

Your "Application form" requests such details. If you would like, we can remove the offending fields and activate your account.

In regards to your intention to collect drivers license, please note that we have strict requirements when it comes to collecting sensitive information: https://www.jotform.com/help/8-Terms-of-Use

"Phishing. You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

Collecting Sensitive Information. You may not use the JotForm Service to collect certain types of sensitive information, including but not limited to, credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm, Inc. including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information."

So, if you will collect drivers license, you must use encrypted forms, make sure to read this guide before enabling this feature: https://www.jotform.com/help/344-Encrypted-Forms-and-How-to-Use-Them

I have now deleted the offensive fields from your form and reactivated your account.

Feel free to get in touch should you have further concerns.

    Great! Thank you so much.

Just so I have understood correctly, I can still collect driver ID submissions but only if the form is set to YES in the encrypt form data option. Once this is enabled a green padlock will be shown at the end of the form next to the submit button. To do this I have to generate a Private key, download and save. When I need to view submissions with encrypted data, I upload the private key which then allows me to view the submitted data.

That is correct, you need to use best security practices to collect sensitive information like Driving License information

When you create a private key, our recommendation is to save it in a place that is both easy to find and you can be sure that it will not be removed. This is an important step since no one will be able to decrypt the data if the private key is lost.

I hope this helps. Feel free to get in touch should you have further concerns.

oh okay. great thanks for all the info!

I have one more question, if I wanted to add the fields monthly budget and deposit amount, would these two fields also need to be encrypted or not? As it is not collecting sensitive data about them but is still seen as sensitive data I wasn’t too sure.

    Hi Noor!

Those two fields are not sensitive data however since your form will be encrypted this includes all fields on the form itself.

    I

cannot log into my account and have not been able to submit a question.  I believe this may be the reason - I may have forgotten to encrypt the form...

After further testing your form I was unable to replicate the issue. I tested the form on an iPhone 8 plus running the iOS 12.4 to check if the form was small on my end and it looked fine.

Did you want the form to look a bit bigger? Is it possible for you to share a screenshot of the form on how it looks on your end?

Oh sorry, I forgot to add when I view it on my website, the form comes out really small and I am unable to click on it at all.

Is that an issue of the website I am embedding my form onto?

I am not sure if I understood you correctly but did you want the from to shall of the questions on one page instead of showing by slides?

If that is the case then the reason it is that way is due to you creating a Card Form which is why only one questions show at a time and will not be able to access all of the questions at the same time.

So instead of the form showing this way:

You want the form to be like this on your site?
