Hi,
Is it possible to make the PayPal amount to be charged dependent on responses to form questions.
For example:
Are you a club member Yes / No
(if "No" then the cost of membership is added to the PayPal total)
Do you require insurance? Yes / No
(if "Yes" then the cost of insurance is added to the PayPal total)

Yes, that is possible. You can use a single choice element for your questions and assign calculation value to each option/answer.
Guide: How-to-Assign-Calculation-Value
Then use the form calculation widget to get the values of each field, please check this guide: How-to-Perform-Form-Calculation-Using-a-Widget
Once you're done with the calculation, you can then pass the total amount to your PayPal payment field.

Hi, I'm not getting the "Calculation" option in the Get Price from option, any ideas please?

Is this the form (https://www.jotform.com/73561910910958) you're working to?
You need to use the Form Calculation widget in order to pass it to the payment field.
In the payment settings, you should choose "User Defined Amount" or "Collect Donations" as the Payment Type. After that, you'll see the "Get Price From" dropdown where you can select the Form Calculation widget.
