Problem in uploading an image in a form

I have set up this form - https://form.jotform.me/92149564597472

The response to the last question in this form is to upload an image (600X400 PX)

There seems to be some challenge in uploading an image there.

Can you please check if I have screwed up in some setting there?

That is an Image field. The Image field is used to add an image to the form, not to allow your users to upload an image. If you want to allow your users to upload an image you need to use a File Upload field.
