Output has changed

My output has changed since we opened the form. The columns have been rearranged and the titles have been removed. We have not changed or removed any questions on the form.  Is there a glitch currently in the system?

We do not make changes to your form. However, you can review the changes made to your form using revision history.

You can also restore the form to its previous state from here. Please check if there are unauthorized changes made to your forms in your absence. If yes, then please change your password.
