The form is not allowing the same user to send more than one form

2. O formulário não está permitindo que o mesmo usuário envie mais de um formulário, preciso corrigir isso. Como Faço?

I notice that the form is enabled with the option 'Unique Submission' using 'Check Cookies and IP' which doesn't allow users to send multiple submissions. 

You may disable it from the form settings and it should be working fine. Please refer to the guide below:
