How can I get simple text fields to show up in the notification email?

    My client is using a form for a job application but when he receives the notification email, the simple text fields do not show up, only fields with questions. 

    He wants to use the reply notification as the official application so it must include some text fields (like equal employment statements) to ensure that the applicant was notified, therefore the field must show in the notification.

    (I have the option to hide fields unchecked, so I am getting all unanswered questions on the notification email, so that's not a problem.)

    Right now, I have worked around the problem by putting all text fields within a question - but it's not a satisfactory solution.

    Please note that the Text fields are not added automatically or not included in the Form fields of the email notifications/autoresponder. However, you may copy and paste the content of the Text fields directly to the email body.
