Spreadsheet to Form Widget

Hi!  I built a form and used the spreadsheet to form widget to help pre-fill the form for my returning customers to use with an assigned username.  I previewed the form and it worked great. I see the widget's field and it is populating with the information I have stored. Whenever I embedded it on my website.  The form only shows the label for the widget not the field to input the unique code. What am I doing wrong?

Please re-embed the form using the iFrame code again. It should resolve the issue.
