Submit button not visible when webpage is refreshed

    I had the form literally go out on the site today - www.arisetheclinicformen.com I did a site using Xara Web Designer and it worked fantastic. The issue was that the form said it went over and then locked up. The info is: https://form.jotform.com/918750038671

    When it refreshed the submit button is not visible now. How do I correct it?

    I have tried accessing the link you provided but was not able to replicate the issue:

    The embedded form is working fine on my end.
