Imported Fillable PDF Form is completely different from the form on the right hand side of my screen

Also, the form that I want to publish and that I am working on is completely different from the form on the right hand side of my screen when I am working on my form.  It is the form from last years event and does not even have the right price or the right options.  Why is that form showing up and not the new one.  Also, on the Upload tab on the top of the form, when I Upload, that form is also last years form and not my current form for this year.  

When you import a fillable PDF form, a web version is created, which will be the one filled by the user. The fields in the web version are connected to your PDF version(show in the right side):
