When a form is submitted and the confirmation email is sent, the name on the email is not anyone associated with our school. How do I change this? Also, the "reply-to" email is incorrect.

Did you delete that submission in your screenshot? I asked because I wasn't able to find it in your form's submission page and there's no email logs of that submission when I searched trwinston@csustudent.net email.

HSMusicReplied on September 27, 2019 at 3:39 AMI guess I must have deleted the submission. I can fill one out again (using my email as the submitter email) and see what happens.
You can see from the email screenshot I have sent that even YOUR email you sent me is addressed to Stephanie Drawdy.

Here are two screenshots showing the wrong address. Stephanie Drawdy is not an employee at our school.

It seems that jotform domain has been named as Stephanie Drawdy. Please go to your email contacts https://contacts.google.com/?hl=en&tab=mC and look for Stephanie and check the associated email.
