Hi @rw710,
If you are referring to the entries that appear when you try to enter data on each field, I believe that's how most browsers work. It happens to me too when I try to fill out your form:
You need to clear your browser's cache and history to be able to prevent data from popping up. You can also refer to this guide on How-to-Disable-and-Clear-AutoFill-Info-in-your-Browser.
If you're referring to a different form, please let us know its link so we could check that too.

rw710Replied on September 27, 2019 at 4:59 PMThanks, I was able to turn off the auto fill in my browser. I have another question.Somehow when I first started using JotForm i came up with a screen that looked like a spread sheet, the whole entry was on one line with all the fields showing. The next submission was on the next line with all the fields showing. They were in a small font but everything was there. I don't know what I did for the submissions to show up like that but would like to do it again but having no luck, any ideas.Bob

It's good to know that it resolves the issue.
Regarding the other concern that you mentioned, I have moved it to a separate thread so we could assist you better on that topic.
I will be posting my response to that in this link below shortly:
