3 product option checkout form with stripe configuration.

I would like to create customs checkout page with 3 product options as below and configure it with my Stripe Account.

You can create such form, for images you can use images checkbox https://www.jotform.com/widgets/image-checkboxes, but you can also set product order form with images: https://www.jotform.com/help/165-Setting-Up-Your-First-Order-Form
