Can we include a digital signature to take consent for form submission?

I am not sure if I have understood your question correctly. Do you want your users to be able to sign in form?

Yes. I would add the signature widget in the form. But for that I need to login on my account. It is still giving the error as a Suspended account. Please help the earliest so that I can login and add Signature Widget.

Your form was suspended because it was asking for some sensitive information from customers.

If you agree, we can remove the fields that violate our terms. Also, please note that some information can still be asked, but you will need to comply by adding two things.

1. Add Terms of use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities.

2. Add a Signature widget where your users sign and agree to provide such sensitive information.

We'll be removing "PAN Card Soft Copy and Aadhaar Card Soft Copy" field as these are file upload fields and can't be encrypted. For "Permanent Account Number (PAN) and Aadhaar Number", make sure to add terms and of use and signature widget for compliance.

We are okay to remove the PAN and Aadhaar card option. Also, will add Terms and Conditions and Signature widget.

I have removed the PAN card and Aadhar card upload fields from the form and activated your account. You may please add the widgets mentioned to avoid getting flagged again.
