I cannot login through my facebook like I normally do

    Please try with another browser as well. That may work. Or, try clearing your browser cache and cookies for Jotform first before logging in to your account.

    If the issue persists after that, try to reset your account password so you can set a new password for your account.

    The account/username dgardner2010 was suspended due to violation to the Terms of Use

    You have forms on your account that capture Passport account data and credit card account data as well.

    You need to provide us proof first that your business require collecting such confidential data using the form.

    If you are capturing Passport data, you must include your company Terms and Agreements clause in the form that the users will be able to see. There should be also Signature field that the users will sign on after they agreed to the your Terms.

    As for the collecting credit card account data, due to our PCI certification, we are required to deny the collection of credit card details on regular forms.


    However, we have other PCI-compliant ways to do this. All you need to do is choose any of the payment processors available in our platform in order to integrate in your forms. You still won't be able to collect credit card details directly though.

    We will only lift the suspension on your account if you agree with our terms and allow us to delete the offending fields mentioned above.

    I do have my terms and conditions listed on each form that the clients agree to and a space for an electronic signature. The payment processor I can switch to my stripe account so that can be removed and I can change it to that instead. I am a licensed and insured Travel Agency so the passport info has to be provided by the client  in order to prevent fraudulent bookings.

    If you don't want us to delete the guilty fields, please send us proof/documents about your Travel Agency. Send the document to reviewers@jotform.com with a subject like of Thread#2074488

    Once our reviewer's team verified the document and if valid, we'll then activate your account.

    However, if you want to reactivate your account immediately, you will need to agree to our terms of use and allow us to remove the offending fields. You can then add the necessary widgets to your forms to prevent further suspensions.

    I went ahead and emailed my documentation over. You may go ahead and delete what it needed to reinstate my account asap so I am not in violation  and I will wait for the okay from the reviewers side before I can collect that information again.

    Please refrain from creating such forms or adding any guilty fields (credit card details) to the forms to avoid any further suspensions on your account.

    My account is locked again. The offensive fields were deleted and I didnt change anything what is the issue?

    The account was suspended due to your form Wanderlust Travel Client Booking Form.

    The form have option to capture credit card account data using plain text fields only.

    These fields are not allowed on the form. You must use Payment integration to be able to collect data using CC account.

    As mentioned before, due to our PCI certification, we are required to deny the collection of credit card details on regular forms.

    Those illegal fields must be removed on your form/s before we can request for consideration to reactivate your account and forms.

    I was told those fields had already been removed I haven't adjusted or added anything to any of my forms in any way since the last suspension. So I was unaware that this was still on the form as it was my under standing it was removed when my account was reactivated.
