Post to PHP not working on specific form

Following the steps in your website, I was able to post data to php forms and get it but when I try it with this form, https://form.jotform.com/91606845815262 I get a 404 page not found error on the same page hat works for my other forms.

This works best with a standalone PHP page. Please try saving your codes that way instead of placing the codes through your WordPress page.
