Need help finding the right form

Hello, I need some help finding the right form to use  I have scrolled through the options but didn't see one that right for  me.  I am a microblading artist and I need a consent form for that.  I have one I already use on paper, is there a way to use my current one but make it in an electronic form?

Looking at the screenshots, I suggest using these fields:

NARRATIVE FIELDS widget - This widget is perfect for such form questions. It allows you to place a narration on your form - with blanks within the narration where your form users will fill out the correct responses.

And for the YES or NO questions, I think you can use an INPUT TABLE field. Please see below example:
