Sharing Jot form

How can I share the jot form I created with other team members and have it go to their email or text if they send to their contacts?  

I only want the form submissions that are on my website or that I text or email the link to my clients to come back to me.  If my team members use that link, how can it go to them when they send to their contacts?

Would you like to send emails of form submissions to your team members or allow them access submissions/modify form or anything else?
