Conditions on my form are not working

For some reason, any conditions used in the form - does not work.

The form had conditions and stopped working properly. I deleted all conditions and then tried to add a very simple one - and does not work. 

Why would this be the case?

As we check your JotForm, there are no conditions set on your form. It seem that you have removed the conditions on your form. Please try adding the conditions and see it works. You may also try reverting the form to an earlier working version from the form revision history.
