Is there a way to change the Thank You page based on condition logic?

Let me ask this... is there a way that after someone completes and submits a form, if certain conditions meet, it automatically sends them to another form? I know there's a way to do it in the email, but not everyone checks their email immediately.  (I don't want to do another form as an iFrame because that can get confusing to people). 

If there isn't a way to take them directly to a different form based on condition logic, is there a way to change the Thank You page based on condition logic?

Yes it is possible to achieve your requirement by taking advantage of conditions feature. You can add condition in form and redirect users to another form based on the user's answer.
