Dear all,
it would be AWESOME :) if you could consider to create the option to have on a page of a multi-page form the option to combine:
- Single-Choice (Radio-Buttons) AND
- Text Fields AND
- a Checkbox with text and the possibility to add a link (e.g. to the Privacy Policy)
as in the attached image.
Currently this is not possible, unfortunately, what means the only option is to create separate pages.

We do have "Multi-line Question" in card form layout but it does not have text filed, checkbox or radio button questions. Unfortunately it is currently not possible to add these fields in cards form layout or multi-line questions.
I would suggest you to please take a look at our "Configurable List" widget and see if that fits into your requirement. It will allow you to add all these questions in one single row. Dear ashwin_d,
thank you for your quick reply. I have tried it, but unfortunately the "Configurable List" widget in a multi-page form:
a) doesnt allow to insert a link (e.g. to the Privacy Policy), so only plain text is possible AND
b) there is no "Custom CSS"-tab, therefore it is not possible to have various rows (but only one).
It would be great if you guys / your developer could think about implementing a feature / widget that allows more configurations as described above (multiple rows, possibility adding a link, a checkbox, text fields, Single-Choice (Radio-Buttons) - all on ONE page of a multi-page form.

May I know if you need it to be on Card Layout? You can customize the classic form to act as one page by customizing the field with CSS. Please note that card form doesn't have CSS, only the classic form.
Let us know if you want to try it with classic form, we'll be able to help you designing your form.

Thank you, Jed_C, for your help. You are totally right, the classic form helps to solve the limitation discussed above. I just found one obstacle:
In the classic form, I cant find the "IMAGE CHOICE"-element. Is it possible that this element doesnt exist in the classic form and if so, could you please add it? That would be AWESOME! :)
Because with this widget, it is possible to upload icons / images directly.

If the IMAGE CHOICE-element isnt available in the CLASSIC FORM, likely the next best option is the Image-ChoiceS-Widget:
Can you tell me what CSS code is needed to put 4 images in one row (currently the maximum is 3 images in one row)?

In order to place the 4 images in one row, please increase the width of the widget from the widget properties. Please refer to this screencast:
