Why my form is linked to someone else?

My buyers have submitted a form for SZ Rolling Acres LLC but it also sent to Oklahoma Standard Poodles. 

I need this fixed. Why is it linked to Oklahoma Standard Poodles. My form should not be sent to these people! 

Please fix this!

I checked your form (81773910950158) and I think you already fixed the autoresponder email template?
