I want to print out an order form that people can fill in at the site

I also would like to have the next issue resolved. I want to print out an order form that people can fill in at the site. When I go to fillable PDF it does not bring up any of the items that can be ordered. Of course, that is the most important part of the form.

To clarify, when you download your submission data as PDF no data is showing? Or are your referring to the attached PDF document?
