Calculation Field Not Working in embedded form

Hi There, there is an error with my form on the live version of my site. For some reason, in the test version the calculated field works, but in the live version not. I have tried everything and it doesn't seem to be working. 

The field is "delivery rate" which should take the distance and multiply it by the delivery rate from the "delivery timeframe" field. 

There seems to be something odd on the embed code within your website.
